Setting Up Application Forms

Application Forms allow you to add custom questions to your candidate flow, giving you deeper insights and helping surface the best applicants faster.

Owner-level setting: Only Account Owners can create and manage Application Forms.

Overview

Application Forms add structure and scoring to the application process. Use them to:

  • Standardize screening
  • Highlight top candidates quickly
  • Reduce time-to-hire

Create a Template

  1. Go to Company Settings > Application Forms.
  2. Click Create New Template.
  3. Enter a template name.
  4. Add questions (see below).

Question Types

Multiple Choice

Fixed options; great for availability or certifications.

Yes / No

Binary checks (e.g., "Are you 18+?").

Checkbox

Multiple selections (e.g., shifts willing to work).

File Upload

Collect resumes or certifications.

Short Answer

Quick open-ended responses.

Optional: Answer Options & Tags

For multiple-choice and yes/no, assign tags:

  • Good — favorable
  • Bad — unfavorable
  • Knockout — disqualifying

Optional: Prompts

Prompts highlight traits like reliability or customer service. AI can analyze responses for these qualities.

Organize Questions

Drag & drop questions into Page 1 or create multiple pages (2, 3, etc.) to group questions logically.

Save Template

Click Save when finished. The template is now available to use.

Apply Application Forms to Jobs

  1. Go to the Jobs tab.
  2. Select a job and open Job Settings.
  3. Toggle Use Application Forms ON.
  4. Select your Application Forms template.
  5. Click Save.

FAQs

Who can create or edit Application Forms?

Only Account Owners.

Does updating a template affect current applicants?

No — only new applicants going forward.
